About the Position We are looking to welcome a Specialist Family Support Practitioner (Mental Health and Disability) to hold primary case management responsibilities, delivering interventions to families addressing identified needs and child protection concerns. The role will also provide advice, guidance, and coaching/mentoring to IFS (Intensive Family Support) practitioners in relation to their area of expertise. Full time opportunity. SCHCADS Award Level 5. Salary range is $97,378 – $101,788 + Superannuation + Salary Sacrifice. Government funded until 31 December 2026 with possible extension. Key Responsibilities: Deliver an outreach model of complex family case-management programs to families, predominantly within their home environment. Maintain a caseload of families and provide evidence-informed case management interventions for clients with varied complex needs specifically, those whose clinical presentations incorporate mental health and/or disability. Engage with families, obtain consent and develop a case plan in conjunction with the family which addresses unmet needs and child protection concerns. Build the practice capabilities of team members by modelling, mentoring/coaching staff during home visits, interventions, complex family interviews, inter-agency meetings and other practice situations. Participate in client case consultations, making recommendations about risk and safety implications as a result of your area of expertise. Minimum Qualifications: Tertiary degree in a relevant discipline (allied health, social work, human services). At least five (5) years’ experience providing complex and intensive case-management and therapeutic services to families, caregivers, young people, and children in a child protection framework. Experience in working with people with mental health, disability, and complex needs; and interacting with families and carers. Cultural awareness and an understanding of how to communicate and work effectively with families from different socio-economic and cultural backgrounds, including Aboriginal and Torres Strait Islander people.
