Yes No Do you have experience working with a neurodevelopmental caseload in a community-based speech pathology service within a multidisciplinary team including working with a broad range of clients from varying socioeconomic backgrounds? Yes No Do you have a flexible and creative approach to providing speech pathology services and support that is responsive to the needs of the community? Yes No Can you demonstrate that you have the ability to partner with service users, including them in goal setting, care planning and care co-ordination to identify and address health and wellbeing goals? Yes No Do you possess a willingness, ability and appropriate levels of health and fitness to travel to and work from remote locations? Yes No Can you demonstrate an understanding of the historical and contemporary context of Aboriginal health including the philosophy and practice of Aboriginal community controlled? Yes No Do you have experience working in an Aboriginal Community Controlled Health Organisation? Yes No Can you demonstrate that you have a comprehensive knowledge of NDIS funding requirements?
