Key Information Position Type: Full time, fixed term Location: Darwin Salary Range: $87,809 – $96,681 (AHP 3.1 to 3.4) Application Closing Date: 23 February 2025 About the Role Danila Dilba’s Transient and Outreach Service is aimed at vulnerable, homeless and transient clients in the Greater Darwin region. As our Aboriginal Health Practitioner (AHP), your role will focus on holistic care provision and support. You will carry out health assessments and address immediate needs while also collaborating closely with your multidisciplinary team to ensure a comprehensive approach to health and social support. Emphasising continuity of care within the Darwin region, you will facilitate smooth transitions for individuals departing the area, or accessing other services, advocating for their health and social welfare. What You Need to Succeed Qualification as a health professional (e.g., Aboriginal and Torres Strait Islander Health Practitioner) and current registration with AHPRA. Minimum three years’ experience working in a Primary Health Care setting. Knowledge of the social determinants of health (environment, economic and social inequalities) that have a profound impact on the health and wellbeing of Aboriginal and Torres Strait Islander people. Well-developed knowledge of, and contemporary experience in, working with clients with chronic health conditions (e.g., diabetes, cancer, respiratory and/or cardiac disease) and relevant management strategies. High level of personal resilience and flexibility with the ability to deal with challenging situations. Demonstrated competence working within multi-disciplinary teams or independently, with strong organisational skills and the ability to plan, manage and prioritise tasks.
