We have an exciting opportunity for a Case Manager to join our Reconnect Team within the Youth team! Full Time Contract until 30th June 2025 (Potential for extension) Who are we? Outcare provides person-centred care for people whose circumstances place them at risk of entry to the criminal justice system, or who are currently serving a sentence. The purpose of Outcare is to support people to regain direction and reintegrate into the community. This role is a community-based early intervention and prevention program for young people aged 12 to 18 years who are homeless or at risk of homelessness, and their families. The role will provide culturally appropriate counselling, mediation, and case management through proactive community outreach. The Role: The Reconnect Case Manager Position will entail undertaking the following: Provide guidance and encouragement to young people to achieve agreed to goals as detailed in their action plan. Encourage positive relationships and support networks through role modelling. Educate, encourage, and support young people to engage with internal and external support services. Support Youth clients to identify short and long-term goals. Establish and maintain collaborative and supportive relationships with youth clients, service providers, other team members and the local community through demonstrating respect, empathy, and the willingness to understand differences in culture and customs. Experience working successfully with at-risk young people experiencing systemic disadvantage as well as young people with justice involvement. Extensive knowledge of barriers facing young people in metropolitan Perth. Extensive experience working with at-risk young people. Understanding of the issues faced by young people in Western Australia. Understanding of homelessness, drug and alcohol dependency, violence and challenging family dynamics of young people. Knowledge and understanding of Aboriginal and Torres Strait Islander cultural customs, and protocols, and the diversity of issues affecting Aboriginal and Torres Strait Islander people in Western Australia, including rural and remote areas. Experience and confidence in delivering mentoring, coaching and modelling to young people. Experience in establishing and maintaining positive and collaborative relationships with partner organisations and other stakeholders. The ability to communicate sensitively and effectively and work well with young people and their families. Other Employment Requirements: Covid-19 Vaccination Certificate Working With Children Check (WWCC) Police Clearance (within 3 months) – A criminal record does not make you ineligible to fill this position. Persons under current correctional supervision will not be considered for this role at this point in time.
