About the role We are seeking a values-driven and proactive Case Manager to join our team! You will be responsible for the delivery of flexible and assertive outreach case management response to young people within the Northern region to receive an appropriate service response to assist them to transition out of the homeless system. Reporting to the Youth Services Coordinator, this is a fixed-term full-time position, contracted for 12 months, working across the Northern region including Fitzroy North and Glenroy sites, VIC. Salary and conditions are in accordance with SCHADS Award Level 5. You will successfully Deliver flexible and assertive outreach case management response to clients with professional and consistent case management to those experiencing homelessness or at risk of experiencing homelessness. Complete comprehensive intake and risk assessment for all young people, incorporating adequate assessment of the needs of accompanying children. Assist young people with a range of activities in preparing for independent living: meals preparation; life and living skills; health and hygiene; recreation and exercise. Participate in duty rosters and actively manage youth entry point by following the intake and assessment procedures. Develop Individual case plans in collaboration with clients and include a range of interventions that address the needs or goals of clients. You will have Degree qualification in social work or related field. Relevant experience in a social service environment is preferred. Knowledge of youth homelessness, crisis intervention, and ability to collaborate with other services within the Northern region. Assessment skills & knowledge of a range of theories including bonding & attachment, trauma, child and adolescent development & strengths-based approaches. A current VIC Drivers Licence. Valid VIC Employee Working with Children Check.
