An exciting opportunity exists for an Aboriginal and Torres Strait Islander Advanced Health Worker, to join the Child Health team on the Sunshine Coast. As a member of the Child Health team, the Aboriginal and Torres Strait Islander Advanced Health Worker role focuses on health promotion, and delivery of culturally safe and appropriate care outcomes for Aboriginal and Torres Strait Islander families and patients aged 0-18 years of age. Sunshine Coast Hospital and Health Service (SCHHS) is in southeast Queensland and extends through the coastal and hinterland areas from Caloundra in the south and Gympie in the north. Key Responsibilities include: Deliver a range of evidence-based interventions in a culturally safe, clinically competent, professional and effective manner including the provision of care coordination, intake assessments, cultural aspects of assessment and treatment plans, direct therapeutic interventions, referrals and cultural support. Establish trust and rapport with children, youth, families and community through strategies to assist in promoting growth, development and overall wellbeing. Provide support for maternal, children, young people and their families, inclusive of appropriate referrals, relevant delivery education, health literacy and using electronic patient management systems. Remuneration value up to $83,804 p.a., comprising salary rates $37.87 - $46.30 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HWF3) (permanent part time positions, hours negotiable).
