Aged Care Assessor/ Clinical Nurse Specialist This position qualifies for a Recruitment Incentive Package up to the value of $10,000 (pro-rata part-time/temporary positions less than 18 months), and a reoccurring Annual Retention Bonus valued up to $10,000 (pro-rata part time), under the NSW Health Rural Health Workforce Incentive Scheme. This scheme recognises the role key health workers play in enabling the delivery of rural and remote health services, and aims to provide incentives over and above award entitlements, to attract and retain staff in these areas. Employment Type: Permanent Part TimePosition Classification: Clinical Nurse Specialist Grade 2Remuneration: $57.11 - $58.98 ph + SuperHours Per Week: 24Requisition ID: REQ545639 Location: Wentworth NSW (24kms from Mildura VIC) What you'll be doing The Aged Care Assessor / Clinical Nurse Specialist will provide clinical assessment services and care planning to determine and manage the needs of frail, older people in the community, residential and hospital settings within the FWLHD. Minimum Requirements: Tertiary qualification & current registration with AHPRA in nursing. Demonstrated post graduate experience working with older people in an acute or community setting. Demonstrated ability to work independently & in an effective manner encompassing appropriate initiative, organisational, problem solving & time management skills. Proven written and verbal communication skills including demonstrated computer literacy. Demonstrated ability to work autonomously as well as function as a member of a multidisciplinary team. Understanding or experience in the health & functional support needs of older people and their caregivers in the community setting. Current Drivers licence (minimum class C) valid within Australia & demonstrated and competent driving ability/experience. Current working with children’s check and aged care check.
