Are you a Care Worker or Case Coordinator with a solid understanding of Care planning and well-developed relationship management skills? We have a 12 month fixed term contract role, with potential for extension, for a Care Coordinator who will also support the Commonwealth Home Support Package (CHSP) Program for a well-regarded NFP in Melbourne. You will be responsible for intake assessments and coordination of aged care clients' care needs, coordinating rosters and ensuring a high level of customer service and quality care. This role would be ideally suited to a candidate who has a strong administration and carer background, or seasoned Coordinators with CHSP or HCP experience. The successful candidates will present with the following: Cert 3 or 4 in Individualised Support/ Aged Care, Diploma of Community Services or higher qualification A strong understanding of the needs of older people and experience working in the sector Outstanding organisational skills and ability to prioritise work effectively Well-developed communication skills and MS Office Suite skills The ability to problem solve and be solutions-oriented A customer-centric approach This role will see you working in a dedicated team, on-site 5 days, based in the Eastern suburbs.
