Aged Care Service Navigator Care Finder Program Do you have a passion to support ageing and vulnerable people? The Care finder role is a great way for you to engage and work with this community. Catalyst Foundation is a not-for-profit organisation and the leading provider of community information in South Australia. Our mission is to help the aged and vulnerable community. We connect people to services and provide relevant, independent information to all South Australians. The Royal Commission into Aged Care Quality and Safety heard throughout its inquiry that the aged care system is complex and some people find it more difficult than others to navigate and access the services they need. It recommended a workforce of care finders be funded to provide face-to-face support to help people navigate and access aged care. The care finder program was announced in the 2021-22 Commonwealth Budget as part of the Connecting Senior Australians to Aged Care Services measure. As a Care Finder, you will provide specialist and intensive assistance to help people within the care finder target population to understand and access aged care and connect with other relevant supports in the community. Some of the functions of a Care Finder include: Supporting people to interact with My Aged Care so that they can be screened for eligibility for aged care services and referred for assessment. Support to explain and guide people through the assessment process including, where appropriate, attending the assessment. Support to help people to find the aged care supports and services they need and connect with other relevant supports in the community. We would love to meet you if you have: Relevant qualifications (e.g. social work, human services, aged care, community services or health) and/or relevant experience. Local community connections and the ability to leverage Catalyst Foundation’s connections with the care finder target population. A detailed understanding and the ability to rapidly attain a detailed understanding of the range of aged care supports and services and other relevant supports that are available in the local community. An understanding of the process to access aged care supports and services, including the steps of this process that are undertaken via My Aged Care and the eligibility requirements for aged care supports and services. A commitment to delivering a person-centred approach that respects and responds to each person’s individual needs, preferences, values and life experiences. The ability to communicate effectively with a broad range of people including a track record to engage in active listening and building support and demonstrating empathy. This role is a 0.6 FTE position and is under the Social, Community, Home Care and Disability Services Industry Award.
