Employment Type: Temporary Full Time, 38 hours per week up until June 2025 Location: SWSLHD District Wide Position Classification: Physiotherapist Level 1/2 Remuneration: $1,407.22 - $2,007.58 per week Requisition ID: REQ553188 Application Close Date: 23/02/2025 Interview Date Range: 26/02/2025 – 05/03/2025 Contact Details: Caitlin Brooks – About The Opportunity SWSLHD Transitional Aged Care Program is seeking a Level 1/2 Physiotherapist to join their community-based Liverpool team (based at Braeside Hospital) on a temporary full time basis. This is an excellent opportunity for a physiotherapist who has knowledge and experience working with a range of aged care and/or rehabilitation populations. TACP is a unique program focused on the reablement of older adults living in the community. It is a multi-disciplinary, community-based program that aims to deliver better health outcomes and maximised quality of life for individuals transitioning into the community following a hospital admission. You will work closely with the multi-disciplinary team and other relevant stakeholders to ensure holistic care for TACP clients and their families. In this role you will be responsible for the assessment of TACP clients in their homes, holistic goal setting, development of client-centered treatment plans and discharge planning. You will require strong clinical reasoning across a diverse range of clinical areas, have excellent communication and problem-solving skills dedicated to achieving improved functional outcomes for older adults. What You'll Be Doing To provide professional and high quality physiotherapy services to patients in SWSLHD. This includes clinical duties (assessment, treatment and education) and participation in professional development, clinical supervision and other relevant departmental activities and administrative duties. Minimum Requirements Current Physiotherapy Registration with the Australian Health Practitioner Regulation Agency (AHPRA). Demonstrated broad range of relevant physiotherapy clinical skills and experience. Demonstrated effective verbal and written communication, computer skills and time management skills. Demonstrated ability to work independently and as part of a multidisciplinary team. Demonstrated commitment to professional development and the application of evidence-based practice. Demonstrated knowledge and involvement in quality improvement activities. Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment.
