Provide expert guidance to support decisions around long-term care and services. Conduct discipline specific assessments, discharge planning and therapeutic interventions to enhance physical function, mobility and daily living skills. Collaborate within a multidisciplinary team to deliver high quality care and achieve optimal health outcomes for clients and their carers.
