Position Summary Working under clinical supervision, the Team Leader oversees a dedicated team of Care Partners, ensuring the delivery of exceptional person-centred care to care recipients. This role leads a team, provides supervision, coaching, and sets clear expectations for best practice. The Team Leader will lead by example, respecting the choices of care recipients, delivering high-quality outcomes, and upholding integrity, transparency, and ethical standards in all actions. This role fosters a culture of ownership, innovation, and wellbeing, prioritising continuous improvement, the safety and empowerment of care recipients and staff, and the responsibility for delivering exceptional care. Additionally, the Team Leader provides operational leadership aligned to organisational priorities, maintaining compliance with aged care legislation and regulatory instruments, and champions Trilogy Care’s inclusive environment. Key Responsibilities Lead and manage a team of Care Partners to provide exceptional telephone-based care management to a portfolio of care recipients. This includes: Monitoring, reviewing, and updating care plans. Educating care recipients on achieving the best person-centred outcomes under the Home Care Package program. Coordinating services. Delivering care management to a reduced caseload while balancing team management duties. Conduct review sessions with team members to provide feedback, coaching, and guidance for professional development. Monitor team and individual performance indicators, ensuring alignment with Trilogy Care’s HR policies and procedures. Analyse data insights to identify trends, address challenges, and develop action plans for improved outcomes. Assist in the review and updating of policies and procedures. Manage direct line duties such as leave and workload management. Ensure compliance with aged care legislation and regulatory instruments, including: Aged Care Act 1997 and 2024. Aged Care Quality Standards. Quality of Care Principles 2014. Act as an escalation point for complex care management issues while supporting resolution efforts. Drive continuous improvement to enhance team productivity and service delivery. Foster a collaborative, compassionate culture focused on accountability, excellence, and person-centred care. Play a lead role in incident management within the team. Maintain workplace health and safety protocols, contributing to a safe and supportive work environment. Support management in implementing organisational strategies and goals. Skills and Qualifications A tertiary qualification or Certificate in Ageing Support is desirable but not mandatory. Minimum 2 years of leadership experience managing and developing teams in a human services organisation or equivalent. Excellent verbal and written communication skills tailored to different audiences and situations. Strong problem-solving and conflict management abilities. Proficiency in computer skills for remote care plan management. Interpersonal skills to mentor and engage team members effectively. Ability to review and analyse policy and regulatory materials for practical application.
