The Case Manager will oversee the care team of specialist professionals in delivering case management for high risk high needs young people. Position Objectives: Provide leadership and coordination of the care team for each young person placed in the Residential Care program. Support young people in the program, and lead the multi-disciplinary team to develop and implement trauma informed plans for each young person. To Be Successful in this Position You Will Need: Tertiary qualifications, preferably in social work / psychology, youth work or equivalent which includes diploma level qualifications. Commitment to providing quality care and experience in leading multi-disciplinary teams. What Makes This Such a Great Role for You: Salary starting at around $97,377 plus super (please pro rata for part time hours). Salary packaging available up to $15,900 plus an entertainment card. SCHCADS award level 5. Part time, 3 days, 22.8 hours. Based at Seymour. Negotiable increments dependent upon experience. Health and Wellbeing programs including a Well Worth It!
