Client Liaison Officer - Aged Care (Home Care) Join All Aged Care in Brisbane as a Client Liaison Officer! Who We Need We are looking for a compassionate, proactive, and detail-oriented Client Liaison Officer to join our team at our Spring Hill office . The ideal candidate will have experience in client liaison, intake, and case management within the aged care sector . This role requires strong interpersonal skills, a passion for client advocacy, and the ability to ensure a seamless experience for clients accessing Home Care Package (HCP) services. Key Responsibilities Act as the first point of contact for new and existing clients, providing clear and compassionate guidance throughout the onboarding process. Manage client intake, assessment, and documentation , ensuring compliance with aged care regulations. Coordinate with the HCP Care Team, Reception, and other stakeholders to facilitate smooth service delivery. Address client inquiries, concerns, and complaints professionally and efficiently . Maintain accurate client records and databases in accordance with privacy laws and industry standards . Educate clients and families on their Home Care Package (HCP) funding and service options . Assist with reports, administrative tasks, and projects as required. Work collaboratively with internal teams to enhance service quality and client satisfaction. To Be Successful in This Role, You Will Have: Experience in client liaison, intake, or case management within aged care, healthcare, or community services. A genuine passion for supporting older Australians. Strong communication, problem-solving, and organizational skills . Ability to engage with people from diverse backgrounds and cultures . Proficiency in MS Word, Excel, and client management systems . Knowledge of Home Care Packages (HCP) and My Aged Care processes (highly regarded). A valid Driver’s License and full rights to work in Australia . National Police Check (or willingness to obtain one). Why Join Us?
