About us:The Sub Acute Chronic Care Rehabilitation team provides interdisciplinary rehabilitative care to clients with a range of complex chronic diseases. These services also work to address unplanned medical admissions and support early discharge by enhancing the quality and availability of community based proactive collaborative care.About the role:The Clinical Nurse Chronic Disease Rapid Access will work in collaboration with the Nurse Practitioner Respiratory and Rapid Access Service (RRAS) to facilitate respiratory patients from the Central Highlands meeting the criteria to access this service virtually. The Clinical Nurse Chronic Disease Rapid Access will also facilitate other Chronic Disease patients from the Central Highlands to access other CQ (Central Queensland) Health Rapid Access Services as these develop and may include but not limited to Diabetes and Renal patients.About you: Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) and a current annual practicing certificate from the Nursing and Midwifery Board of Australia (NMBA). Proven ability to provide clinical education to patients and carers and support and demonstrate own ongoing professional development. Ability to comprehensively assess, plan, implement and evaluate patient care. Developing ability to lead activities required as part of contribution towards the CQ Health Nursing and Midwifery Professional Practice Model.
