The Team: The Community Nursing Liaison Clinical Nurse Consultants work on a rotating roster, seven days a week 0730-1600 within the Royal Hobart Hospital, located in the Integrated Operations Centre (IOC). The Clinical Nurse Consultant (CNC) is a community nurse, linked with the Hobart Community Health Nursing Service. The Role: The CNC will work across the acute hospital to identify patients who can be cared for safely in the community, prompt early referrals and facilitate a smooth transfer of care through improved communication. You will work collaboratively with acute hospital providers to build the knowledge of the advanced skill set in the community nursing sector, and the types of care that can be managed by community nurses to provide ongoing care and support to patients, carers, and families. You will need: Demonstrated high level clinical knowledge with an understanding of primary health care principles. Demonstrated high level communication skills with ability to collaborate effectively within multidisciplinary teams. Demonstrated effective organisational skills and ability to problem solve to achieve positive health care outcomes for patients. Details of appointment: Permanent, part time, shift worker position, working up to 32 hours per fortnight, commencing April 2025. *notwithstanding hours to be negotiated with the successful applicant. Successful applicants will be required to meet the essential criteria: Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse Registration to Work with Vulnerable People Applicants should note the following criteria are desirable: Hold or is working towards a relevant post graduate tertiary qualification Current Driver’s Licence Applicants should note, the position features: Work across the Southern region acute, sub-acute and community nursing services, working predominantly within the Royal Hobart Hospital and based within Southern Hobart Community Health Nursing Team Salary: $114,851 to $121,886 pro rata per annum + our Employer 11.5% superannuation contribution is on top of this amount. Salary Packaging: You’ll have access to salary package a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible.
