Clinical Nurse Consultant Nurse Navigator South West Hospital and Health Service has an exciting opportunity for a Clinical Nurse Consultant Nurse Navigator to join our Primary and Community Care team in Roma. Join us in building better health in the bush! The Clinical Nurse Consultant Nurse Navigator is accountable at an advanced practice level for the provision of specialised knowledge and advanced clinical practices to a cohort of patients within the rural/remote context of the South West Hospital and Health Service. The Nurse Navigator role will utilise advanced clinical skills and professional decision making to deliver a safe and efficient patient journey across the care continuum through the reduction of service fragmentation, educating and empowering individuals and coordinating patient care in a positive way.
