Clinical Nurse (Hospital in the Home)

Job Description

This role provides expert nursing care for patients receiving medical care at home, while coordinating services and supporting a multidisciplinary team. An opportunity has become available for several Clinical Nurse's (Hospital in the Home) to join Gold Coast Health (GCH) where the focus of our 12,000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia. You will Provide advanced nursing care for patients in their home environment Coordinate care delivery, ensuring optimal patient outcomes and standards Deliver in-home clinical procedures such as wound care and cannulation Participate in quality improvement initiatives and evidence-based care About you To be successful in this position you will require: Bachelor of Nursing or equivalent Registration with the Nursing Midwifery Board of Australia Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B Tuberculosis screening, to identify if the tuberculosis vaccination may be required Declaration of serious discipline history must be disclosed Criminal History Screening will be required for new employees (GCH will cover cost) About Us Hospital in the Home (HITH) services provide multi-disciplinary team care which aims to deliver safe, reliable and efficient client and family focused health care services, primarily in community settings including patient homes, workplaces, with some centre-based service delivery. HITH services include adult HITH and Children's Hospital in the Home (CHITH). HITH is a hospital substitution for the Gold Coast Hospital and Health Service (GCHHS) with the aim of providing medical and nursing care to adults in their home environment for conditions requiring medical and nursing intervention.
