Job Description

The Hospital In The Home (HITH) team operates collaboratively with acute inpatient hospital services, community-based services, residential aged care facilities, and multidisciplinary Allied Health services to provide evidence-based care to patients who meet specific criteria within the West Moreton Hospital and Health Service HITH catchment. The HITH team also provides care in the community setting by reviewing patients with acute illness in their place of residence, who would otherwise be required to remain in hospital in an acute setting to receive the same care. This role is required to work varying shifts across a 7 day a week roster, including public holidays. Hospital In The Home (HITH) operates a 7-day service, inclusive of weekends and public holidays. The Registered Nurse in the HITH service provides acute, sub-acute and post-acute nursing care to patients within their home setting. This role supports the general function of the unit through the application of the principles of best practice, clinical decision making and professional development. The registered nurse supports excellence in nursing care and patient outcomes, is a competent clinician who provides nursing care with clinical competence and capability, optimising access to care services. The registered nurse works in partnership with the multidisciplinary team to provide person and family centred, quality, safe patient care. The Registered Nurse HITH provides contemporary, evidence-based nursing services and demonstrates competence in the provision of nursing care as specified by achievement of the standards of practice and registration requirements from the Nursing and Midwifery Board of Australia (NMBA). This is evidenced by educational preparation, continuing professional development, adhering to relevant legislation and the context of care. About you Our ideal candidate will be someone who can demonstrate the following: Contemporary clinical knowledge in the acute nursing within the hospital and community settings, delivering competent nursing care in complex situations while providing support to other team members. Ability to comprehensively assess, plan, implement and evaluate nursing care. Demonstrated ability to provide patient and family centred care to the patient using best practice and within the standard of practice of a Registered Nurse. Demonstrated ability to communicate to a high standard effectively and engage with members of the multidisciplinary healthcare team, patients, relatives, and visitors whilst displaying a high level of customer service. Demonstrated ability to educate, inform, counsel and support patients and families on general and clinical information relevant to patient's care, accessing the Clinical Nurse when expert advice is required. Demonstrated ability to build and maintain strong relationships, open communication, and a positive environment within a multidisciplinary team, actively contributing to the learning and development of others to a high standard.
