The Opportunity Core to delivering on Amplar’s growth strategy is our Community Care Team Coach (CCT) which focuses on “Making Health Personal” for both our people and our clients. The Community Care team delivers a range of health care services including nursing, allied health, personal care, meals and domestic services, transport and accommodation services. The Community Care Team Coach plays a pivotal dual role within our service delivery ecosystem. They are responsible for guiding, supporting, and empowering teams to operate with high autonomy, while also taking on essential management functions to navigate the operational complexities of our healthcare system. Working as part of a very experienced team, you will work across 4 key areas: Empowering and Guiding Teams: The coach enables teams to self-organize around patient care, encouraging them to make decisions, solve challenges, and collaborate effectively within their scope of autonomy. Coordinating Operational Needs: Given our systems requirements (such as payroll, rostering, scheduling, and financial approvals), the coach handles specific administrative tasks and coordinates with central functions to ensure seamless operations. Ensuring Quality and Efficiency: The coach monitors quality indicators and ensures that care standards and efficiency metrics are upheld, integrating best practices across teams. They act as a liaison between teams and the broader organization. Cultivating a Hybrid Culture: In this hybrid model, the coach balances their role as a mentor with operational oversight, ensuring teams feel both supported and empowered. The successful candidate needs to live in Adelaide Western . This is a full time (1.0 FTE) position. The ideal candidate will have a passion for leading, coaching, and mentoring a team. They also have a strong customer service orientation and relationship management. Additionally, the candidate will bring: You must be a Registered Nurse Div 1 or Allied Health professional (PT/OT/POD/Dietetics/EP) with current registration with a professional body. It is desirable that the candidate holds tertiary management qualifications or equivalent work experience relevant to the role. Evidence of recent professional development, to further enhance leadership and management competencies. Demonstrated experience in human resources and financial management within a complex health care service. Demonstrate depth knowledge of current clinical practice, its delivery and models of care. Proficient computer skills, including experience using multiple systems and the ability to provide clear and concise plans and reports.
