We have an exciting opportunity for a dedicated Manager to oversee our new Single Assessment Service and support the ongoing development of our Plan Management Team. About the role The Coordinated Care Services Manager is responsible for the effective management, leadership, and coordination of the Plan Management and Single Assessment Service within CatholicCare. As the Manager you will play a pivotal role in driving the successful implementation and operation of the Single Assessment Service as well as overseeing and growing the Plan Management team to deliver high-quality, client-focused services. These commercially driven programs require the delivery of high-quality, client-centred services while ensuring financial viability and sustainable growth. The primary objectives of this position include: Lead and manage the Coordinated Care Services Team, including recruitment, supervision, mentoring and performance management; Coordinate daily operations of the team, ensuring effective allocation of resources and workforce; Oversee the delivery of person-centred services, ensuring they are tailored to the individual needs of clients while promoting quality of life; Ensure adherence to legislation and accreditation standards related to the program within the purview of the role; Develop and maintain relationships with key stakeholders, including Centacare Brisbane, health services, service providers, community organisations and government agencies, to facilitate seamless referrals and service integration. Additionally, this role provides expert guidance to the team to ensure services meet clinical and quality standards. Core Competencies Substantial verbal and written communication skills, with the ability to engage effectively with clients, staff, and external stakeholders; Demonstrated experience in leading and developing a team, utilising strong interpersonal skills to recruit, train, supervise and foster a collaborative, supportive work environment; High level of ability to interpret data and performance metrics to monitor service quality and to make informed operational decisions; High level of ability to manage multiple priorities and maintain high standard of service delivery within tight timeframes; High level of cultural competency, with the ability to provide services to clients from diverse backgrounds; High level of experience in the use of applicable corporate systems and client management systems, including the Microsoft Office Suite. Qualifications and Experience Tertiary qualifications as a Registered Nurse, Occupational Therapist, Physiotherapist, or Social Worker and AHPRA registration (or equivalent registration authority); 5 years’ experience leading a team preferably within the Aged Care, Community Health or Social Services Sector; Current Driver’s license (QLD); In-depth understanding of the aged care assessment processes; Knowledge of the aged care system or NDIS system in Australia. The successful candidate will also be required to demonstrate entitlement to work in Australia, and to disclose any pre-existing injury or disease which may be adversely affected by undertaking the inherent requirements of the position. All offers of employment are subject to the satisfactory completion of relevant screening and background checks, including an Australian Federal Police check, an NDIS disability worker screening check, and attainment of a Blue Card for working with children, prior to commencement.
