Job Description The Opportunity We are seeking a dynamic and passionate Credentialed Diabetes Educator (CDE) or working towards credentialing to join our supportive multidisciplinary team on a short-term contract - part-time. Responsibilities include but are not limited to: Deliver individualized diabetes education and management plans tailored to clients’ needs, goals, and preferences. Provide education on diabetes self-management, including blood glucose monitoring, insulin administration, nutrition, physical activity, and the use of diabetes technology. Collaborate with a multidisciplinary team, including General Practitioners, Dietitians, Exercise Physiologists, Mental Health Clinicians, and other allied health professionals. Work across different ages and irrespective of gender, race, religion or political persuasion. Work independently and as a member of multi and interdisciplinary teams. Develop an effective, client-centred therapeutic relationship with clients/families/carers, by listening effectively, sharing information, and understanding client and other partner’s needs. Liaise and collaborate with external partners to improve service provision. Maintain accurate clinical documentation and contribute to continuous quality improvement activities. Desired Skills and Experience Registered nurse or allied health professional with a post-graduate qualification in Diabetes Education. Current Credentialed Diabetes Educator (CDE) status with ADEA or working towards credentialing. Possess knowledge of current and evidence-based Diabetes Education practices and relevant clinical practice guidelines. Experience in diabetes education and management across diverse client populations. Experience in developing, implementing and evaluating individual and group programs. An understanding of Diabetes chronic disease management models to promote self-management. Understanding of current diabetes technology and ability to support clients in its use. Exceptional communication and interpersonal skills to engage with clients, families, and stakeholders. Client-centred approach, with a commitment to empowering individuals to take control of their health. Ability to work independently and collaboratively within a multidisciplinary team.
