About the Organisation The Victorian Aboriginal Health Service (VAHS) was established in 1973 to address the specific medical needs of Victorian Aboriginal communities. 2023 marked a landmark year as VAHS proudly celebrated its 50th year of providing a comprehensive range of medical, dental, allied health and social services for our community. As well as providing a variety of medical services, VAHS is committed to supporting the well-being of the community through contributions to community events and activities. About the Opportunity The dietician will work within a multi-disciplinary team environment and will be responsible for delivering a quality nutrition and diabetic service, with high standards of care to clients of VAHS, through the application of competent clinical skills in health education and promotion and preventative health strategies. Culturally appropriate services will be provided on site, in the home and via Telehealth as appropriate and will be supported by Aboriginal Health Workers where required. The position will work across a range of program areas including Commonwealth Home Support Program (CHSP) and the Home and the Community Care Program for Younger People (HACC PYP), Home Care Packages and NDIS packages. Key responsibilities Provide high quality and timely primary care nutrition and dietetic services to VAHS clients in accordance with agreed targets. Work in collaboration with other clinicians internally and externally, in a multi-disciplinary approach to ensure culturally sensitive services are delivered at all times. Provide home visits/ outreach services as appropriate to meet the needs of the target group. Maintain accurate and timely client records in client management systems that always meet reporting requirements. Record timely and accurate clinical information on client records in My Aged Care portal in accordance with strict compliance measures. Prerequisites Relevant Tertiary qualifications in nutrition and dietetics. Membership of the Dietitian’s Association of Australia with full APD status OR eligible for full APD status. NDIS Screening Check. Victorian Working with Children’s Check. National Police Check. Current Victorian Driver’s License.
