Home Care Package - Case Manager – Service Consultant Murray Mallee Aged Care Group Inc/ My Choice Care The Opportunity: Murray Mallee Aged Care Group Inc. (MMACG) is looking for an enthusiastic Full Time Service Consultant to join our existing metro team, working in our Adelaide office. The Case Manager/ Service Consultant will work collaboratively with the team to support elderly community members living at home to reach their goals and maintain their independence. This role involves: Coordinating services for older people Implementing interventions with older people at risk Working collaboratively with other departments including admin, rostering, and finance. Facilitating individual service planning and delivery Managing legal and ethical compliance Coordinating client-directed services Fantastic documentation and time management skills This role involves some travel within the Metro Region; however, fleet vehicles are provided. This role is mainly an in-office position with occasional visits to clients' homes. Minimum Requirements: Current Police Clearance Minimum of 2 COVID-19 vaccinations Sound computer skills Empathy and understanding towards the needs of older people Preferred Minimum 1 year of recent work experience in aged care. The salary for this role is set at Level 4 of the Social, Community, Home Care and Disability Services award. About Us: Established in 1994, Murray Mallee Aged Care Group Inc. (MMACG) is a non-profit in-home aged care organization that strives to deliver affordable, quality, and person-centered care to elderly individuals in the Murraylands, Riverland, Southern Fleurieu, Alexandrina, Adelaide Hills, and Metropolitan Adelaide regions.
