SESLHD is committed to enabling our community to be healthy and well, and to providing the best possible compassionate care when people need it.The social worker will provide high quality care and clinical expertise and specialised knowledge when planning, coordinating and implementing care for the patient cohort consistent with NSW Health, South Eastern Sydney Local Health District (SESLHD) policies, procedures and standards. The social worker will have responsibility for providing clinical care and care coordination to people across SESLHD who have end of life and palliative care needs who are in the last 12-24 months of life. The social worker will support these patients through the Community Supportive Care Services (CSCS) model at St George Hospital (SGH). This position is suitable for clinicians interested in palliative care. The position will assist with triage of patients to the appropriate community or outpatient psychology services. Once accepted to the CSCS, the social worker will provide comprehensive health assessments, therapy, education and support to patients and their families. Within this role, there will be a requirement to undertake clinical reviews across clinic, community and virtual care settings, either alone or in conjunction with a multidisciplinary team. The successful candidate will be supported by the Palliative Care Teams within the Supportive Care Service at St George Hospital as well as by the social work team. In addition to the above, there will be opportunities to lead education and research activities and undertake strategic planning and capacity building activities for this service to identify unmet palliative care and end-of-life needs, promoting early referrals to specialist palliative care services, and supporting communication between the ward-based clinician and the palliative care multidisciplinary team (MDT).Benefits: Salary packaging options that reduce your taxable income and increase your take-home pay! Minimum Requirements: Qualifications in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award and eligibility for membership of the AASW. Comprehensive post graduate clinical experience in the provision of social work intervention with clients with end of life or palliative care needs. Excellent negotiation, decision making and advocacy skills including the ability to interact constructively and collaboratively with a diverse range of stakeholders, throughout all organisational levels. Ability to provide in-depth advice and education to enhance service delivery and to guide and support others in providing service excellence, with the capacity to provide clinical supervision to other staff. Ability to work as part of a multi-disciplinary team and to apply advanced reasoning skills and independent professional judgement when dealing with situations of a complex or critical nature. Ability to initiate, lead, complete and share quality improvement initiatives and service evaluation processes and lead ongoing quality improvement activities with other staff. Demonstrated commitment to ongoing clinical and professional learning. Current NSW Class C Drivers Licence or equivalent and ability to travel as required.
