Central Coast Local Health District has an exciting opportunity for a Social Worker Level 3 to join the Community Allied Health Team. The position of the Senior Social Worker exists within Community Allied Health (CAH), as a part of Central Coast Local Health District. This role is part of the social work team within CAH, but has an identified area of specialty skills and knowledge in the area of Palliative Care. This role will be responsible for developing and enhancing the service provided by CAH social workers and other staff, to patients with complex needs who are approaching or reaching the end of their life. About You You have a degree in Social Work from an accredited course providing eligibility for membership of the Australian Association of Social Workers. You have extensive clinical experience in broad and diverse psychosocial assessment, interventions, care planning, individual and family counselling, grief, loss and bereavement counselling, and group work, across a diverse and broad range of diagnoses and conditions, in particular those approaching or reaching the end of their life, within the community setting. You have the ability to prioritise work, have a flexible approach to clinical and non-clinical demands, and manage and respond to stressful situations. Where You’ll Be Working Social Work services are provided in the community setting involving home visits, clinic and group interventions. This will require travel to locations throughout the Central Coast region.
