Where You’ll Be Working The position of Social Worker is part of a community-based multidisciplinary allied health team. Community Allied Health provides a community service approach seeing clients and their families across the Central Coast. This position will work within a dedicated interdisciplinary team to deliver high-quality care in a dynamic team environment.Primarily the position is to provide assessment and counselling to assist patients and their carers to deal with the emotional impact of illness and to undertake case planning, liaison and advocacy and referral to ensure access to appropriate community services. The social worker is to work closely with interdisciplinary colleagues and senior managers to ensure the development and delivery of effective and proactive social work services. The social worker will be expected to be active in the areas of service development, service evaluation and staff and student teaching.This position is required to work collaboratively with other social workers within the team to ensure appropriate and timely service provision across all geographical sites and services. The clinical duties are performed across multiple sites within CCLHD. There may be a requirement to work weekend or public holiday shifts, if required.
