About You You have a degree in Social Work from an accredited course providing eligibility for membership of the Australian Association of Social Workers and hold a current driver’s licence (overseas qualifications must supply the AASW Overseas Assessment number). You possess demonstrated application of extensive clinical experience across a diverse and broad range of diagnoses and conditions within the community setting (including identified skills detailed in the advertisement and task list). You have demonstrated skills in team management and evaluation, both within social work teams and multidisciplinary teams, and demonstrated understanding of the current AASW Practice Standards as they relate to the position. Where you'll be working The position of Senior Social Worker is part of a community-based multidisciplinary allied health team. Community allied health services provide a community service approach seeing clients and their families across the Central Coast. This position will work within a dedicated interdisciplinary team to deliver high quality care in a dynamic team environment providing social work intervention to support clients in the community, as well as many specialist teams across CCLHD (such as Transitional Aged care programs, Specialist Palliative Care Service, chronic disease programs & Integrated Care Co-ordination team). The position plays a critical clinical and supervisory role and is responsible for the delivery and monitoring of Social Work services to clients referred to the service. The primary clinical component of the position provides assessment and counselling to assist patients and their carers to deal with the emotional impact of illness and to undertake case planning, liaison and advocacy and referral to ensure access to appropriate community services. The social worker is to work closely with interdisciplinary colleagues and senior managers to ensure the development and delivery of effective and proactive social work services. The social worker will be expected to be active in the areas of service development, service evaluation and staff and student teaching. The senior social worker, working with the other senior social work staff, are responsible for the supervision, training and performance appraisal of Level 1/2 social workers within the service, as well as other relevant social workers as directed by Director-Social Work. The senior social worker is also responsible for managing and facilitating effective service provision and enhancement of the Social Work service within the team. Social Work services are provided in the community setting involving home visits, clinic and group interventions. This will require travel to locations throughout the Central Coast region.
