Direct message the job poster from iCoordinate Disability Services Director at ICoordinate Disability Services About Us iCoordinate Disability Services is a boutique service provider offering Support Coordination, Specialist Support Coordination and Psychosocial Recovery Coaching. Our close-knit team works collaboratively to provide person-centered, holistic, coach-approach to Support Coordination for NDIS participants. Due to the high demand for our service, our team is growing, and we now have Three Full Time Positions available for a passionate, like-minded individual to join our existing team based in Maitland. This role is to support our participants in Maitland/The Hunter, Port Stephens, Newcastle, Lake Macquarie, and surrounds. About the Role Supporting Participants to understand their NDIS plan, goals and objectives whilst promoting choice and control and supported decision making. Coordinating mainstream, informal and formal supports with Participants to assist them in achieving their goals. Establishing a respectful and collaborative working relationship with Participants and their family/support network that encourages capacity building and promotes choice, control, and inclusion. Developing strong and collaborative working relationships with Mainstream Stakeholders including all Government Departments such as Department of Communities and Justice, Health, Education, Employment along with Community NFPs and other NDIS Providers to ensure effective communication, referrals, and support coordination can occur. Assisting Participants where there are points of crisis or escalation of needs. Regularly monitor NDIS plan funding to ensure services are being delivered within the Participants' goals and allocated budget. Engage with NDIA regional contacts, including Local Area Coordinators. Compliance with iCoordinate Disability Services values, code of conduct, policies and procedures and relevant government legislation and standards where relevant. Maintain confidentiality of Participants, Carers, Families and the Organisation. Skills and Experience 1-2 years experience as either a NDIS Support Coordinator, NDIS LAC and or Planner is highly regarded. Relevant qualification (e.g. Degree in Human/Social Services, Education or Allied Health, Diploma/Cert IV in Disability Services, Mental Health or equivalent in any of the above). Demonstrated commitment to the rights and interests of people with disability. An in-depth knowledge of the disability sector. A keen understanding of the NDIS key principles. Demonstrated ability to be client-focused and work collaboratively with Participants, Carers, Families and Stakeholders. Established time management, financial budgeting, problem solving and conflict management skills. Independence and proactive learning style and sound problem-solving skills. Strong knowledge of mainstream services, community groups and community resources and good community networks. Proven skills in the following areas: oral and written communication, research, problem solving, record keeping, evaluating and reporting outcomes. Ability to prioritise workload and work independently and as part of a team. Computer Literacy in word, excel, database, and email software. Demonstrated ability to think creatively and problem solve. Ability to meet deadlines in a high demand work environment and to exercise judgement and initiative. A current driver's licence. A NDIS Worker Screening Clearance - NDISWC. A current WWCC. NDIS Code of Conduct training Certificate. What we offer Competitive remuneration.
