Your Opportunity: The Nurse Navigator Service is a state-wide model of care designed to provide holistic support to patients with complex health conditions requiring an expert level of healthcare coordination. This integrated and proactive approach to health care service delivery will help ensure patients receive the most appropriate service when and where they need it. Nurse Navigators work across system boundaries to actively advocate and improve health literacy, in close partnership with patients and families, other health and allied-health services to improve the patient's healthcare experience and quality of life. The Nurse Navigator role employs advanced clinical and professional practice capabilities to deliver a positive patient-centered health experience for individuals. This role aims to facilitate a safe and efficient patient journey across the care continuum through the reduction of service fragmentation, educating and empowering individuals, and coordinating patient care. The Nurse Navigator must demonstrate capabilities that support the effective coordination of patients with complex care needs across all specialty services, focusing on those with multiple co-morbidities and high-frequency presenters for unscheduled care. What we're looking for: Mandatory possession of a nursing qualification from a registered tertiary institution or completion of an approved hospital training program as a Registered Nurse and Registration with the Australian Health Practitioner Regulation Agency with a current annual practicing certificate is essential. Appointment to this position requires proof of qualification and if applicable registration or membership with the appropriate registration authority or association. Certified copies of the required information must be provided to the appropriate supervisor/manager prior to the commencement of clinical duties. Possession of context-specific postgraduate qualification in nursing is highly desirable. Preferred Qualifications: Mandatory possession of a Medical qualification from a registered tertiary institution and Registration with the Medical Board of Australia (Australian Health Practitioner Regulation Agency) with a current annual practicing certificate is essential. Appointment to this position requires proof of qualification and if applicable registration or membership with the appropriate registration authority or association. Certified copies of the required information must be provided to the appropriate supervisor/manager prior to the commencement of clinical duties. At least 3 years postgraduate hospital experience is highly desirable. Why join us?
