About the Role This opportunity is for two full-time Nurse Practitioners, working fixed term until June 2025, to provide coordination of comprehensive care of consumers referred to the Primary Health Clinic (PHC). This role includes identification and management of all relevant comorbidities in this complex cohort including drug dependence treatment, blood borne and sexually transmitted diseases, physical, psychological or psychiatric health needs, along with social and legal issues. For complex clients, the Nurse Practitioner forms the core of the complex case planning and management team, with less complex clients being stabilised and referred to other practitioners, with or without opiate pharmacotherapy. About Us The Primary Health Clinic sits as part of Monash Health Drug and Alcohol Service, which is a service within the Mental Health Program of Monash Health committed to providing opportunities for change and growth for people affected by problematic alcohol and drug use, improving their physical and mental health, and reconnecting them to their family and the community. The Primary Health Clinic provides pharmacotherapies, drug overdose response, specialised health care, basic nursing and mental health support, general dietary advice, as well as other practical supports for people who inject drugs. Key Accountabilities The Pharmacotherapy Nurse Practitioner would be both an extension of existing services and allow new service capacity to be delivered through the Monash Health Primary Health Clinic with a focus on medically, psychiatrically and behaviourally complex clients who use drugs. About You To ensure your success in this role, you will bring the following demonstrated skills, experience, and knowledge: Appropriate tertiary qualifications in a discipline such as Nursing, Social Work, Occupational Therapy or Psychology Current registration as a Nurse Practitioner with AHPRA Membership of professional nursing bodies/organisations Post graduate qualification in related field (mental health and/or drug and alcohol studies) Current Victorian Driver's License #J-18808-Ljbffr
