Grade 2 Physiotherapist – Heart Failure Virtual Ward and Rehabilitation Fixed term, Full Time; 80 hours/fortnight until Feb 2026 - Maternity leave backfill The Role: An exciting opportunity exists for an enthusiastic Grade 2 Physiotherapist with a commitment to quality, patient-centered care and sound clinical skills to join Northern Health. Northern Health Allied Health is a well-established team with a strong focus on quality improvement activities, evidence-based practice, and patient-centered care. The Grade 2 Physiotherapist will provide leadership and ensure high-level, evidence-based physiotherapy care to patients managed in the Heart Failure Virtual Ward and Heart Failure Rehabilitation across Northern Health. This program aims to transition heart failure patients from an inpatient admission to ‘hospital in the home’ under the care and monitoring of the Heart Failure team. The aim of the program is to reduce the length of inpatient stay and reduce readmissions within 30 days, whilst delivering best practice care. To be successful in this role you will have: A background which includes significant clinical experience including general medical and cardiorespiratory physiotherapy. An appreciation of physiotherapy services in an organisational context and as a senior staff member, the ability to take responsibility for leading and managing resources to meet the requirements of Northern Health. Experience and/or interest in clinical education and staff supervision. Demonstrated capability in service innovation and development. Commitment to health promotion and health independence.
