The aim of the Equity Model is to pilot a continuity of care stacked interventions service model to reduce inequities and improve health and development outcomes of families from priority population groups in Bankstown. The role will lead service integration internally and cross-agency collaboration to build awareness and understanding of the program and a genuine commitment to integrated care across the health and social care systems. This role will have a particular emphasis on women with perinatal substance use and/or a disability and will be responsible for ensuring strategies are in place to have the greatest possible impact for these population groups. The role will have operational, clinical and strategic governance for the program and associated clinical team and will work with professional leads within the District to support good professional governance. The role will play a role in informing policy and strategy related to antenatal care and early childhood health and development care in SWSLHD. Post graduate qualifications in a relevant health field and/or extensive work experience in the area of women and children (including antenatal and child and family services). Demonstrated experience in leading a clinical service, and in developing systems to ensure effective operational, strategic and clinical governance. Demonstrated understanding of the policy and research frameworks that underpin healthcare equity and the practical application of these at the Local Health District level. Demonstrated high level proficiency in planning, implementing, and evaluating healthcare services or projects and in leading change management processes. Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment.
