Position Title: Senior Housing Services Worker Classification: Dependant on Qualification & Experience Salary per hour: Dependant on Qualification & Experience Employment Type: Ongoing full-time Award: Allied Health Professionals (Victorian Public Sector) (Single Interest Employers) Enterprise agreement 2021-2026 The Senior Housing Services Worker will lead a team of housing service workers to provide case management, crisis intervention and outreach services to clients that are either homeless or at risk of homelessness. The Senior Worker will hold a reduced caseload and will use a range of crisis interventions and case management approaches to support people to access housing. You will: Facilitate and support referrals to the program. Support the Team through operational supervision, providing secondary consultations, reflective practice, opportunities for debriefing, and joint work with clients when appropriate to enhance best practice. Liaise and network with other staff and service providers to develop a multidisciplinary approach to addressing the needs of clients. Facilitate client centred integrated care through assessment of needs and referral to other relevant Programs and services. Provide effective casework and support to clients within an integrated framework. Maintain required level of service hours and systems for the collection of statistics in accordance with funding body requirements. Maintain and review client records. Participate in the professional education of students on placement as required. Be accountable for monitoring brokerage expenses & flexi-package applications. Participate and contribute to relevant professional Networks. Participate in the development and/or review of relevant Policies and Procedures. Participation in and ensure compliance with relevant Accreditation Standards. Contribute to service integration, team development and Dhelkaya Health strategic goals. About you Tertiary Qualifications in Social Work, Community Services or related disciplines. Demonstrated skills and experience in housing, family work, case management and cross-sectoral collaboration. A commitment to working from a Social Model of Health with an Integrated Team approach. Key Selection Criteria A qualification in Social Work, relevant Community Services or equivalent. Demonstrated leadership and high-level communication skills. Demonstrated skills and a high level of experience in the housing and homelessness sector. Ability to work from strength based and solution focused frameworks. Demonstrated understanding of the Social Model of Health. Ability to demonstrate highly developed interpersonal and communication skills with a collaborative and co-operative approach to relationship building with partners and agencies. Demonstrated skills in case plans and report writing. Excellent knowledge of Central Victorian community based and Government networks. Sound organisational, analytical and time management skills. Excellent written and oral skills, including demonstrated ability to write and present clearly and concisely for other professionals and clients. Ability to work as part of a team.
