We are looking for an experienced Manager to join our collaborative leadership team in Community Aged Care within the Care Management team. The ideal candidate will have a proven track record in leading high-performing teams and supporting team members in their professional growth and development. This key role will involve monitoring service delivery to ensure that consumers are empowered to set goals and implement strategies for independence, wellness, and reablement. What can you expect to be doing? Provide strategic and operational leadership in the provision of care management services provided by a high functioning team. Lead the Care Management team through significant change within the Aged Care Sector. Ensure the Care Management function provides integrated care across other Community Aged Care functional areas and with other business areas. Ensure team compliance with defined Key Performance Indicators, reporting trends and recommending and leading responsive actions. Support team leaders to develop and reach their full potential. Monitor service delivery to ensure consumers are encouraged to identify goals and strategies for independence, wellness and reablement. Plus much more utilising your strong leadership skills! What do you need to bring? Proven experience in leading a team, being compassionate toward employees, providing support, encouragement, and recognition. Proven experience in a senior service delivery and management role in the health and/or community sector. Applied knowledge and understanding of the priorities, key policies, legislation and guidelines relevant to Community Aged Care and understanding of the health and wellness of older people and leading and implementing care coordination and/or case management. Knowledge and linkages with health and community services, networks, peak bodies, business linkages and resources Demonstrable ability to manage a large and diverse workload, set priorities and meet deadlines, balance potentially conflicting demands of service provision. Tertiary qualification in health and/or human services discipline, community development, community services, social planning, social sciences or equivalent relevant experience. Tertiary qualification in Nursing or Allied Health (desirable). Who is AnglicareSA?
