Subacute Allied Health Intake Coordinator Subacute Allied Health Intake Coordinator Inpatient Rehabilitation Ward 80 hours per fortnight Full Time, Permanent ERH services the Shire of Campaspe and the cross-border Murray River Council. Applications are sought from suitably qualified Allied Health Professionals to take the next step in their career and join the Inpatient Rehabilitation team as the Subacute Intake Coordinator at Echuca Regional Health. Key responsibilities include: Actively collaborate with the Bed Manager, NUM and Clinical Health Lead to develop and maintain streamlined intake systems for the Inpatient Rehabilitation Ward Effectively assess referrals and coordinate admissions to the Rehabilitation Ward Assess the individual patient’s fit with the admission and discharge criteria for the relevant care setting and identify the patient’s potential to achieve goals in a timely way Attend Case Conference to determine and plan upcoming bed availability and assist with Discharge planning as required on Subacute ward – with allocated patient KCP role Actively participate in quality improvement activities to ensure a streamlined, efficient and patient centred service delivery during subacute intake and transition of patients to the Rehabilitation Ward Actively participate in meetings as requested Qualifications/experience required: Tertiary Qualifications in an Allied Health Profession Current registration with relevant registration body where applicable Minimum 5 years’ experience in an Acute or Subacute setting Demonstrated experience in care coordination, patient assessment and patient centred goal setting skills NDIS Worker Screening Check Working for ERH At ERH we offer a supportive and inclusive culture that offers opportunity for growth and development.
