Job Description

The role of the Support Coordinator is most integral in the delivery of a high level of quality supports for individuals with complex needs. The principal purpose of the role is to ensure that Participants are completely satisfied with the service delivery, that complex programs are delivered with a high level of support worker to client engagement, and that participants are on track to achieve their goals as articulated in their NDIS plan or Care Plans. About you Your key responsibilities will include but not be limited to: Providing the coordination of supports to complex programs in allocated portfolio; supporting the individual’s ability to connect with informal, mainstream, and funded supports, and to increase capacity to maintain support relationships, and resolve service delivery issues. Ensuring the quality of supports is in line with best practice and in line with allied health partner, Support Coordinator / Case Manager recommendations. Provide crisis management and Incident reporting follow up. Facilitating regular team meetings and attending care team meetings for participant programs in allocated portfolio. Completion of critical incident review processes, including participating in and documenting investigations as required. Personal Attributes/Selection Criteria 2 years+ experience in a similar position within the NDIS industry and a solid understanding of Disability Act. Experience with Psychosocial Disability Coordination. In depth understanding of NDIS systems/ PRODA as well as budgeting and plan review services. Relevant tertiary qualifications. Ability to build rapport with clients and effectively communicate with various people from industry organisations. Excellent problem-solving skills. An ability to thrive in a fast-paced customer service environment including the ability to manage multiple and competing priorities. Excellent written and communication skills. Excellent computer literacy and high level of administration skills. Key Performance Indicators Actively encourages, engages and supports clients to make independent decisions. Models person centered approaches for other team members; providing appropriate guidance and support. Respects and contributes to each client supported to achieve their goals. Engages with service providers to facilitate goal achievement. Ensures consistency of support with the developed Client Support Plan. Action referrals in timely manner. Relevant qualifications in the field Previous Experience as a Support Coordinator Relevant Tertiary Certification Recent Cleared National Police check (No later than one month from interview date) NDIS Screening Check NDIS Worker Orientation Module CPR & First Aid Certification Current Employee Working with Children Check (WWCC) Driver’s license (for roles where travel is required to deliver services) Must satisfy all visa requirements for working in Australia 100 Points of ID COVID Vaccination Certificate (Fully Vaccinated) If you are passionate about providing quality care for our Support Coordination Participants, have experience in the NDIS Sector, have experience with plan review and management of coordination and have the drive to complete all the above mentioned responsibilities and duties to an exceptional standard, become a Support Coordinator at Sunrise2Sunrise.
