Role details The Care in the Right Setting team at the Mackay Base Hospital is seeking applications for a Team Leader to join their team on a temporary basis. Your opportunity To provide leadership, management, and direction to various administrative staff within the Care in the Right Setting service, ensuring the delivery of such services in a professional and proficient manner whilst ensuring that operational effectiveness is maintained. About The Team The Care in the Right Setting service focuses on ensuring that our patients receive the right care, in the right setting to improve patient experience and outcomes. This service encompasses Hospital in the Home and Virtual Ward – hospital substitution services with the expressed aim to provide medical, nursing, and allied health care to patients in their home environment. Hospital in the Home and Virtual ward utilize remote patient monitoring (digital platform) in a hybrid outreach / Remote Patient Monitoring model of care. Without these services, eligible patients would otherwise be admitted to hospital for monitoring/treatment in a traditional inpatient bed. Other services in Care in the Right Setting include Interim Care Beds and the Watchhouse Model of Care in addition to our Geriatric Evaluation Management/Rehabilitation – Hospital in the Home, Geriatric Emergency Department Intervention service and Residential Aged Care Facility acute care Support Service which aim to improve outcomes for our older patients through concentrated processes to enhance older persons care in Residential Aged Care Facilities and the Emergency Department, support to Residential Aged Care Facilities improved continuity of care for older patients' post-discharge.
