We are recruiting up to full time (37.5 hours) Wellbeing Assistants, to join the Healthy Knowsley Service. You will have a set shift pattern between the hours of 08:00-20:00, Monday to Friday, to be agreed between service and post holder - example working pattern shown below. The role of Wellbeing Assistant focuses on supporting individuals to access appropriate Lifestyle Services based on an assessment of their needs. This needs assessment is completed via telephone. The role will involve occasional travel across the Trust therefore, the post holder must be able to meet the travel requirements of this post. Main duties of the job Wellbeing Assistants work with individuals at the start of their health and wellbeing journey, supporting them to access relevant lifestyle support that meets their needs and preferences. Wellbeing Assistants discuss the range of non-clinical lifestyle options available to the individual and make the onward referrals based on choice made. This role is part of the Healthy Knowsley Service and provides a positive and personalised experience for the individual. The aim of the role is to support individuals to access the Service that can best support them to make lifestyle changes. You will be expected to perform the following duties: Communicate with individuals about promoting their health and wellbeing. Interact with individuals using the telephone as the main method of communication. Agree courses of action following assessment to address health and wellbeing needs of individuals. Advise on access to and use of services. Provide information to individuals, groups and communities about promoting health and wellbeing. Discuss individual preferences and needs. Work in partnership with others in the team and wider community. Job responsibilities To support the delivery of individual care interventions under the direct supervision of an appropriately skilled practitioner. To support the provision of routine advice to clients and carers in relation to identified needs and the wider public health remit of the Health and Social Care organisation under the direct supervision of an appropriately skilled practitioner. To maintain accurate, clear, concise and contemporaneous records as per trust policy and communicate information to members of the Team as necessary. To be aware of the role limitations of practice and when to seek further advice from a supervising appropriately skilled practitioner. Provide concise feedback and information updates to other members of the Team as required. To demonstrate a knowledge of external agencies who could be involved in patient care and work in partnership with these agencies under the direct supervision of an appropriately skilled practitioner with regards to follow up and provide feedback. To participate in the maintenance, ordering and security of stocks, supplies and equipment as required. Person Specification Qualifications Level 2 Health/Social Care qualification (equivalent to NVQ level 2) or evidence of equivalent experience of working at that level in a health/community related field. English and Maths GCSE. Knowledge/ Experience Understanding of barriers to adopting a healthy lifestyle. Interest in reducing health inequalities. Knowledge of lifestyle factors and impact on health and wellbeing. Understanding of the impact of the wider determinants of health on choices related to lifestyle. Experience of building a rapport with members of the public. Experience of using IT packages. Ability to work independently and as part of a Team. Understanding of relevant Public Health Agenda. Able to use initiative. Understands own limitations. Committed to improving health. Experience of interacting with members of the public during promotional events. Excellent communication skills (written and oral). Experience of working in a health improvement setting. Skills Ability to build effective relationships with service users whilst maintaining professional boundaries. Excellent communication skills to support confidence building. Awareness of limitations of the role. Ability to deal with safeguarding issues effectively, under supervision. Ability to maintain professional boundaries. Ability to work as part of a team and autonomously when required. Flexibility to meet the demands of the Service. Ability and willingness to travel in the course of performing duties.
