Job Description About the Aged Care Assessment Officer (Comprehensive - Clinical) role Reporting to the Coordinator (Single Assessment Service), this position manages aged care assessments, builds tailored care plans, and fosters client partnerships to promote independence. Managing a diverse caseload, this role supports service improvement and offers fixed term part-time, full-time, or casual roles. Your Passion and Commitment will see you: Conduct comprehensive/ clinical client assessments (home, face-to-face, phone) for My Aged Care referrals. Partner with clients to set goals, develop support plans, and facilitate referrals. Maintain client data and records, ensuring funding compliance. Manage a diverse caseload, supporting clients, including those with dementia and from diverse backgrounds. Stay updated on government guidelines and ensure accreditation compliance. To succeed in this role you will have: A qualification in a health-related discipline, such as Allied Health, Social Work, Nursing, or another relevant specialist area related to health or aged care. Current, unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA) or another relevant professional association. Demonstrated experience in delivering services in aged care settings, such as geriatric evaluation, rehabilitation, palliative care, or community nursing. This includes experience working with people living with dementia and in community settings, including CALD communities. Sound understanding of My Aged Care Portal and knowledge of the Integrated Assessment Tool (IAT).
