We believe that’s what makes us truly different! We have an exciting opportunity for an Aged Care Navigation Specialist to join our Central Services Team. Based in the South West, this role will have a focus on admissions to our three Aged Care Homes in Bunbury, Dalyellup and Eaton. In this role, you will provide information, support and advocacy to customers as they navigate the complex aged care system. Our Aged Care Navigation Specialists work with customers to understand their needs, identify available options and make informed choices about their care. This role is offered on a permanent full time basis, working 38 hours per week across Monday to Friday. Flexible working arrangements are supported, with a mix of site based and work from home available. As this role supports multiple locations, it’s important to note that regular travel between sites in the South West region will be required, with the occasional visit to Perth. A travel allowance will be provided to you in support of this. About You Demonstrated ability to develop relationships with customers / potential residents, their families or representatives and staff Excellent customer service skills with a passion for the wants, desires and needs of older Australians Demonstrated understanding of sales processes, budgets and sales targets The ability to contribute to meeting sales targets and maintaining healthy occupancy levels A working knowledge of Home Care funding programs, Medicare claims, AN-ACC classification and the My Aged Care Service and Support Portal is desirable Excellent verbal and written communication skills A strong understanding of the Microsoft Office suite, as well as database / CRM software The ability to work independently and autonomously, but also as part of a team Willingness to uphold the Mission, Vision and Values of The Bethanie Group Willingness to obtain relevant vaccinations Current National Police Clearance (or willingness to apply for one) The Role Provide customers with the information and assistance they require about the aged care system, including eligibility requirements, available services and supports and accessing My Aged Care Service and Support Portal Support informed decision-making regarding the delivery of care and services across the continuum of aged care Proactively manage enquiries with the view to convert enquiries into sales while maintaining compliance with internal Bethanie systems Foster relationships with respite residents and their families to optimise re-admission and permanent conversions Manage waitlists and vacancy boards in Epicor (our CRM system), including working closely with Home Care, Village and Facility Managers, including looking at Home Care as an interim option for customers in the waitlist Engage and develop strong relationships with internal teams including Admissions Coordinators, Case Managers and other healthcare professionals, to facilitate and optimise the onboarding process Monitor and communicate changes in legislation that affect subsidies and / or funding Review the National Screening and Assessment Form (NSAF) and / or Integrated Assessment Tool, including identifying the prospective AN-ACC classification, and highlighting any concerns in the delivery of care and services Working with Bethanie Many of us work because we want to make a difference.
