Strong networking with other CNCs with community and district services. The Clinical Nurse Consultant (CNC) is an expert Palliative Care practitioner who, through leadership, facilitates evidence-based practice. The Palliative Care CNC will promote a service which fosters clinical excellence, research, and professional leadership. The Palliative Care CNC contributes to quality health care by collaborating with health care providers and consumer groups, developing clinical practice standards, and evaluating their outcomes. The Palliative Care CNC is responsible for providing services which facilitate effective management of care in the given specialty. The position works with the community health nursing team and the multidisciplinary team across the district in a consultative model. What You'll Be Doing The Clinical Nurse Consultant (CNC) is an expert Palliative Care practitioner who, through leadership, facilitates evidence-based practice. The Palliative Care CNC contributes to quality health care by collaborating with health care providers and consumer groups, developing clinical practice standards, and evaluating their outcomes. The CNC is responsible for providing services which facilitate effective management of care in Primary and Community Health. Current Authority to Practice as a Registered Nurse with the Australian Health Practitioners Regulation Agency (AHPRA). At least 5 years full-time equivalent post-registration experience, with at least 3 years full-time experience in palliative care. Approved post-graduate nursing qualifications in a field relevant to palliative care or such other qualifications or experience deemed appropriate by the organisation. Demonstrated evidence of strategies used to maintain current knowledge of clinical practice advances and research in the relevant specialty. Demonstrated ability to apply expert knowledge to provide safe, compassionate quality care that is culturally responsive. Demonstrated evidence of the use of critical thinking as it relates to the delivery of advanced evidenced-based nursing care. Demonstrated ability to effectively communicate and engage with patients/consumers, carers, and the multidisciplinary health care team to identify and ensure responsiveness of care needs. Applicants must possess an Australian C-Class driver’s license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain an NSW C class (P1 or P2 included) license within three months of appointment.
