About us:CQ Health is offering an exciting and innovative opportunity for a Clinical Nurse Consultant (CNC) to work within the Community Specialist Palliative Care Service (CSPCS). This service encompasses Rockhampton, Capricorn Coast and Mt Morgan, Gladstone, and Banana (based in Gladstone) and the Central Highlands (role based in Emerald). The service team comprises of a Staff Specialist Palliative Care, Clinical Nurse Consultant, Clinical Nurse, Social Worker, and Administration Officer and operates collaboratively with General Practitioners and non-government organisations and across CQ Health. About the role:The key functions of this CNC role include: Using advanced palliative care nursing skills and knowledge. Work in partnership with community care providers in the provision of specialist palliative nursing care and symptom management. Work as part of the multidisciplinary team to provide specialist palliative care interventions and care planning. Facilitate timely assessment, care planning, implementation of interventions and evaluation, ensuring quality outcomes for consumers. About you:Registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) with a current annual practising certificate.A relevant postgraduate Diploma or Masters' degree is an expectation of this position. Where the successful applicant does not currently hold this qualification, commitment to completion within an agreed timeframe is expected and will form part of the performance and development plan.
