About the role Become part of a dynamic and supportive team that values your contributions and encourages your professional development. In this role, you'll play a key part in helping participants achieve their goals by coordinating services in alignment with NDIS plans, while ensuring compliance with NDIS standards. Enjoy consistent daytime hours, Monday to Friday, from 9 AM to 5 PM, with no after-hours or on-call responsibilities. The Calvary Home Care office is conveniently located in the Adelaide CBD. Your main duties will include: Comprehensive Assessment and Care Planning - Working with participants to develop tailored, person-centred plans ensuring participants receive the right supports tailored to their needs and goals. Monitoring Progress - Regularly reviewing and updating care plans to reflect changes in participants' needs, goals, or circumstances to achieve the best possible outcomes. Budget Management - Assisting participants in managing their NDIS funding effectively, ensuring they maximize their independence and support of their plan without overspending. About you NDIS Experience: You have previous experience working with NDIS participants and a solid understanding of NDIS plans, funding, and regulations. Strong Case Management Skills: You hold excellent organisational and problem-solving abilities to manage multiple cases, develop and review plans, and ensure services are delivered in accordance with NDIS requirements. Excellent Communication: You are skilled at building relationships with participants, families, support coordinators, and other stakeholders, ensuring open communication and effective service delivery. Empathy & Advocacy: You are committed to advocating for the rights and needs of participants, ensuring they have access to the services and resources required to achieve their goals. Attention to Detail: You maintain a high level of accuracy in documentation and reporting, ensuring compliance with NDIS standards while tracking participant progress and outcomes.
