About the Role: Join our team as an Exercise Physiologist Grade 2 – Community on a part-time (0.60 EFT/45.6 hours per fortnight) basis based at the Monash Health - Pakenham Health Centre. Community Rehabilitation Exercise Physiologists provide care to adult clients across a number of clinical streams, including (but not limited to) neurological, musculoskeletal, general, cardiac, oncology and pulmonary rehabilitation. You will work collaboratively with a fantastic group of people who are inclusive, passionate and share a vision to provide excellent, evidence-based care for patients and their families. About Us: The Monash Health Community Rehabilitation Service actively engages with people to improve and maintain health, independence and well-being. We do this by coordinating and/or providing integrated, multidisciplinary care across multiple settings within the local community, inclusive of home and centre based therapy. Our aim is to help clients live their best lives by enabling them to access the support they need, when they need it. The Exercise Physiologist works closely with the interdisciplinary Community Rehabilitation team to provide a therapy program that is both holistic and client centred.
