Family Support Practitioner - Identified Role $67,922 - $73,356 + Super + Salary Sacrifice To perform this role it is essential that the incumbent is an Aboriginal or Torres Strait Islander person. About Us The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region. IUIH’s Birthing and Early Childhood Services (BECS) aim to support women, infants and families, from preconception and early pregnancy through to early childhood (up to three years of age), to enhance family functioning and promote childhood development and parental wellbeing. Within BECS is the Birthing in Our Community (BiOC) service, the Australian Family Partnership Program (AFPP), and the Deadly Fit Mums exercise program. The BiOC program was developed and implemented under First Nations leadership, the service provides women with access to a known midwife and a family support worker throughout their pregnancy, birth and up until baby is three years of age. The service is supported by an Indigenous workforce strategy and is based out of a culturally-safe and welcoming Aboriginal Community Controlled community-based hub. About the Role Integrated within a family services model grounded within a culturally intrinsic parenting model which supports connection, belonging and cultural growth. The purpose of this position is to provide social and emotional support and health education to women and family members throughout the woman’s pregnancy and up to three years following the birth. Also, to work effectively with other social and health practitioners to identify and access appropriate services and supports to meet their family’s needs. Day to Day Duties: Create a pathway for culturally respectful engagement between women, midwives, and other service providers. Work closely with women and her family to identify her social emotional wellbeing needs to ensure optimal outcomes for mother and baby. Facilitate access for women to attend antenatal appointments as arranged by MGP midwives/obstetrician. Conduct psychosocial assessment, collaboratively develop and maintain social emotional wellbeing plans. Provide support in the instance of pregnancy loss or traumatic birth. Current Criminal History Check or willingness to obtain. Current C Class Drivers Licence (Qld). Willingness to travel between IUIH sites as required. Desirable, but not mandatory: Demonstrated experience working with Aboriginal and Torres Strait Islander people and/or communities. What Can We Offer?
