The Federal Government appointed Country SA PHN to enhance the delivery of focused and appropriate health services to the rural communities of South Australia. The goal of the organisation is to improve the efficiency and effectiveness of health services for people particularly those at risk of poor health outcomes, improve the coordination of health services, and increase access and quality support for people. Job Description The Role Country SA PHN (CSAPHN) commissions local health and primary care service providers to deliver multidisciplinary team (MDT) care for underserved rural populations, including Aboriginal and Torres Strait Islander peoples, people who are financially disadvantaged and people who are experiencing chronic illness. Within an MDT care model, planning and coordination occur with a general practitioner and a range of allied health and nursing disciplines to deliver comprehensive and organised care to meet as many of the patient's needs as possible.The MDT Project Lead will be responsible for supporting the design, commissioning, implementation and evaluation of multidisciplinary team care projects across CSAPHN. The MDT Project Lead will be expected to: Lead the design, project planning, implementation, and coordination of activities with internal and external stakeholders to ensure the considered delivery and monitoring of projects relating to MDT are consistent with Commonwealth guidelines and CSAPHN expectations. Initiate engagement with key stakeholders to co-design project activities, including general practices, allied health clinicians, community service providers, clinical councils and community advisory committees. Identify gaps and opportunities to leverage and integrate activities, create a better experience for patients and clinicians. Establish reporting processes supported by data collection and data management practices. Build a comprehensive toolkit to support general practices, IPHCS service providers, allied health and key stakeholders. This position is part of the Health System Integration and Innovation team. It is expected that this position will remain flexible to meet the changing needs of the organisation and the community. The MDT Project Lead will be responsible and accountable for the development, implementation and evaluation of MDT projects and services, through the following key responsibilities in alignment with CSAPHN’s Strategic Priorities : Priority 1: Commission effective and efficient services that delivery health equity Contribute to Country SA PHN planning and commissioning through identification of community needs related to the implementation and evaluation of MDT projects and services. Priority 2: Facilitate local capacity for the delivery or responsive, sustainable services Work with governance bodies, peak bodies and general practice to design, implement and evaluate MDT project activities. Identify and provide timely quality advice on issues affecting the project and facilitate decisions leading to proactive project implementation, including identifying emerging pressures, issues, risks and initiating management strategies to address them. Support the enhancement of current commissioned services to develop multidisciplinary team care where appropriate. Prepare internal and external communications, including project resources, promotional communications, status updates, briefings and reports, to a professional standard. Priority 3: Improve people’s experience of primary health care Develop and maintain collaborative partnerships and work with all levels of internal and external stakeholders. Represent CSAPHN in the implementation, establishment and evaluation of MDT projects and services. Establish meetings and working groups in accordance with project requirements, including administrative activities and reporting. Priority 4: Practice responsible and effective corporate and clinical governance Lead the development, documentation and maintenance of project implementation and evaluation plans. Implement discrete activity management, including monitoring and reporting on discrete activities to key internal and external stakeholders. Support operational planning through established governance processes and translate this to project goals and activities. Manage priorities and agendas to achieve agreed objectives, key milestones and timelines. Capacity to work both autonomously and in a self-managed team environment with an agile and fast paced mindset to achieve project milestones. Other duties: Proactively work in a way that directly supports CSAPHN strategic priorities and objectives. Engage with internal and external stakeholders at all levels with respect, collaboration, and cultural sensitivity. Commit to embedding reconciliation initiatives into business and our everyday work in line with our CSAPHN Reconciliation Action Plan. Ensure best practice processes across all areas of responsibility and identify opportunities forinnovation and continuous quality improvement. Regularly attend team and staff meetings and participate in working groups. Comply with reporting requirements as directed. Perform other reasonable duties as required to meet organisational objectives. Attend and present at meetings, seminars, forums when required. Collaborate with CSAPHN staff, Clinical Councils and Community Advisory Committees as required, sharing information and providing input within the confines of the position. Desired Skills and Experience About You Qualifications: Relevant tertiary qualifications in a health-related area and/or extensive experience in health-related project support and delivery. Experience/Knowledge/Skills/Abilities: Demonstrated ability and skills in project management, including contributing to project planning, project documentation, monitoring, discrete activity management and delivery to outcome. Strong understanding of multidisciplinary care team activities and approach. High level analytical, problem solving and planning skills. Excellent interpersonal skills and ability to develop and maintain strong collaborative partnerships and create harmonious working relationships. Strong communication skills both verbal and written and ability to communicate with a range of stakeholders (internal and external). Ability to be self-directed and demonstrate high-level time management skills. High-level information technology skills and strong working knowledge of Microsoft Office. Strategic thinking with demonstrated comprehensive knowledge and understanding of health service delivery and within the South Australian health context.
