Role Description The Service Coordinator will be responsible for tasks related to participant services, including Supported Independent Living (SIL), Community-based activities, and Short-term accommodations. This position is an integral part of our management team, as the Service Coordinator will play a pivotal role in ensuring smooth service delivery, maintaining compliance with NDIS guidelines, and managing stakeholder relationships. Key Responsibilities Service Delivery : You will be responsible for overseeing the delivery of services to our participants. This includes assisting participants with the specific support they require to meet their individual needs. Problem Solving : Addressing barriers to engagement, ensuring smooth communication with the participant and their respective care team. Compliance & Standards : Ensuring that all services in Melbourne are delivered in accordance with NDIS guidelines and maintaining compliance with quality and safety standards. Team Leadership : Working closely with a multidisciplinary team, promoting a culture of teamwork and excellence. Reporting : Providing regular reports on service delivery metrics, participant feedback, and any operational challenges to senior management in Melbourne. Rostering : Maintaining and updating the participant's care plan and rostering system. Budget Management : Collaborate with the accounts team to ensure services are delivered within budget in Melbourne and identify opportunities for financial efficiency.
