About You Do you have qualifications eligible for membership of the Australian Association of Social Workers; or qualifications recognised for registration with the Occupational Therapy Board of Australia? Registered Nurse - with the Australian Health Practitioner Regulation Agency (AHPRA), relevant post registration qualifications and at least 3 years' experience working in the clinical area of their specified post-graduate qualifications or such other qualifications or relevant equivalent experience; or qualifications eligible for appointment as a Counsellor or Welfare Officer. What You'll Be Doing IMHAOD Declaration Each person's unique journey of recovery will be supported by mental health drug and alcohol services in a way that fosters hope, purpose and resilience. IMHAOD Statement of Intention The intention of the MNCLHD Integrated Mental Health and Alcohol and Other Drugs Services (IMHAOD) is to provide recovery-oriented, trauma informed services that are guided by evidence-based practices and collaboration. The Transition and Physical Health Coordinator is primarily responsible for two primary functions: Within the wider multidisciplinary team to coordinate, negotiate, and manage discharge planning needs of consumers to facilitate a smooth transition from hospital to the Community Mental Health Service and coordinate 7-day post discharge follow up clinics.
